Insurance Codes

Most dental insurances will cover bone graft surgeries to varying degrees. Please contact the insurance provider for an accurate estimate. See below for a guideline of insurance codes that can be used:

D7140 – Extraction, non-surgical, erupted tooth or exposed root
D7210 – Surgical (Extraction) removal of Erupted tooth requiring elevation of mucoperiosteal flap and removal of bone or section of tooth
D4266 – Guided tissue regeneration – resorbable barrier
When submitting the claim, it is good to include SOAP (Subjective, Objective Assessment Plan).
